RN Caregiver Support Santa Rosa
The Staff Nurse in the Caregiver Support Program serves a pivotal role in tracking, scheduling and completing the in-home caregiver and Veteran assessments at required intervals and for the documentation of the assessment in CPRS. The nurse will ensure coordination of care through warm hand-offs and follow up with the PACT team, assigned Behavioral Health service provider at SFVAMC and at the CBOCS (s) and with the CSC. The Nurse will initiate orders and referrals for access center appointments for those Veterans and caregivers seeking Mental Health Services. Certain visits will involve longer travel and occasional overnight stays throughout the SFVAMC catchment area. If training and background permits, the nurse will provide counseling services. The nurse will serve as the POC for nursing within the caregiver support Program and will participate in future CSP development. The nurse will have opportunities to educate others in the VASF about the caregiver program and play a role in engaging other Veterans and their caregiver in the program. Work Schedule:

In cases of graduates of foreign schools of professional nursing, possession of current, full, active and unrestricted registration will meet the requirement of graduation from an approved school of professional nursing. OR The completion of coursework equivalent to a nursing degree in a MSN Bridge Program that qualifies for professional nursing registration constitutes the completion of an approved course of study of professional nursing. Students should submit the certificate of professional nursing to sit for the NCLEX to the VA along with a copy of the MSN transcript. The physical requirements of this position include lifting, prolonged standing, frequent bending and stooping, walking, reaching and repetitive motion. heavy lifting (45 lbs and over); heavy carrying (45 lbs. and over); straight pulling (up to 1 hour); pushing (1-2 hours); reaching above shoulder; use of fingers; both hands required; walking (up to 3 hours) standing (up to 3 hours); repeated bending (up to 1 hour); ability for rapid mental and muscular coordination simultaneously; near vision correctable at 13 to 16; hearing (aid permitted). The mental/sensory requirements include recall, reasoning, problem-solving, hearing, speaking clearly, writing, keyboarding, reading and logical thinking. The environments' pace can vary from steady to critical, requiring handling of multiple priorities, frequent, sometimes intense customer interactions and the ability to adapt to frequent changes in a some-times noisy environment. There may be the need to use personal protective equipment to prevent exposure to disease, illness and hazardous materials. HCP are individuals who, during the influenza season, work in VHA locations where patients receive care or who come into contact with VA patients as part of their role.
