The CBOC operates under a PACT Model (Patient Aligned Care Team). Together with his or her team, comprised of a PCP, RN, LVN, and a Clerk, the PCP cares for a panel of approximately 700 patients. The PACT provides comprehensive care for our Veteran population, including the diagnoses, care, and treatment of both acute and chronic conditions. Common diagnoses encountered include diabetes, hypertension, chronic obstructive pulmonary disease, degenerative arthritis, depression, congestive heart failure, and chronic pain. Core duties to include:
obtaining patient histories, performing physical exams, formulating a working diagnosis and developing/implementing a treatment plan according to clinical guidelines; determining appropriate hospitalization, and/or disposition to Nursing Home Care, Assisted Living, Community Nursing Home, and/or Home Care. Ordering lab tests, x-rays, EKG's and other diagnostic procedures. Initiating or expediting requests for consults, special tests, or studies. Providing education and counseling to patient and family in preventive care, medical conditions and use of prescribed treatment and drugs. The incumbent will assess, stabilize, and determine disposition of patients with emergent conditions consistent with medical staff policy regarding emergency and consultative call services. The physician may collaborate with/oversee mid-level providers such as nurse practitioners, and works collaboratively with other specialties in and outside of the VA system to manage or co-manage patients' health. Select mental health and subspecialty services are available on-site. Other mental health, medical and surgical subspecialties services are available through consult to the San Francisco VA Medical Center or to the community. Work Schedule:

United States Citizenship:
Non-citizens may only be appointed when it is not possible to recruit qualified citizens in accordance with VA Policy. Degree of doctor of medicine or an equivalent degree resulting from a course of education in medicine or osteopathic medicine. The degree must have been obtained from one of the schools approved by the Department of Veterans Affairs for the year in which the course of study was completed. Current, full and unrestricted license to practice medicine or surgery in a State, Territory, or Commonwealth of the United States, or in the District of Columbia. Completion of residency training, or its equivalent, approved by the Secretary of Veterans Affairs in an accredited core specialty training program leading to eligibility for board certification. Experience:
1. Board certification/eligibility in internal medicine or family practice 2. Experience as a primary care physician in the VA System 3. Experience with an electronic medical or health record Reference:

Applicants must meet physical standards for the position. A physical examination prior to placement is required. This is a designated drug testing position. After appointment, you will be subject to random testing for illegal drug use. Requires lifting 15-44 pounds; pushing (approx. 2 hours); reaching above shoulder; use of fingers and both hands; walking and standing from 3-5 hours and kneeling. Ability for rapid mental and muscular coordination simultaneously. Must have depth perception and ability to distinguish basic colors and shades of colors. Hearing aid is permitted.

The fraudster will send a check to the victim who has accepted a job. The check can be for multiple reasons such as signing bonus, supplies, etc. The victim will be instructed to deposit the check and use the money for any of these reasons and then instructed to send the remaining funds to the fraudster. The check will bounce and the victim is left responsible.
