Resident Services Coordinator Job
Summary:
This is a line staff position within one of our residential programs at Nation's Finest.
The RSC is primarily responsible for the day-to-day activities and safety of the clients, as well as the coordination of services to be provided.
This position works closely with other staff members onsite to provide the best services possible to all residents.
The RSC is not responsible for lease up file compliance.
Job Duties:
Responsible for maintaining a calm, friendly and cooperative atmosphere within the facility and with the participants to promote a clean, orderly and healthy environment at all times.
Responsible for ensuring timely entry of client data into HMIS system for the location in which the RSC works.
HMIS entry required for all permanent supportive housing units within the project.
Conducting intakes and assessments of clients for the permanent supportive housing program.
Works in tandem with HUD VASH case managers to implement necessary programs/referrals for tenants.
Maintains logs of maintenance issues to be addressed by maintenance staff, and reports on any ongoing maintenance issues to Site Director.
Document client progress and other significant events in individual client case files according to agency standards and as requested.
Promptly and accurately report any issues or concerns regarding clients to direct supervisor or designee.
Maintain strictest confidentiality with regard to protecting client's privacy and record keeping.
Manage a caseload of clients and develop an Individual Treatment Plan (ITP) or Individual Service Plan (ISP) with the tenant, in accordance with agency standards.
Ensure clients have a clear understanding of the goals and action steps outlined in their ITP/ISP and receive support in pursuing those goals.
Provide case management services to program clients, including (but not limited to):
crisis intervention, individual and group case management services, and community referrals.
Utilizing curriculum and materials available, facilitate group sessions as needed/directed.
Conducts checks units in compliance with local/state law, outside grounds and common rooms to ensure clients are involved in program activities or otherwise productively engaged in ITP/ISP tasks.
General supervision of the tenant activities onsite, and enforcing safety rules.
Participate in staff meetings, house meetings, and the Client Review Team (CRT), as necessary.
As a member of the CRT, coordinate with other staff to ensure clients are getting quality, fully integrated services.
Support other case management programs as instructed and coordinate attendance and progress with other case managers.
Support clients by providing one-on-one sessions addressing job skills, skills upgrades, adult education, and other job and education-related referral services.
Support clients by providing referral services and warm hand offs to local EDD representatives to assist with gainful employment.
Provide one-on-one and group assistance to clients who need assistance with life skills training.
Provide event coordination and event set up for clients who would like to participate in social and recreational services such as exercise, group events, gardening, move nights, etc.
Provide referrals for clients who need legal assistance.
Refer clients who need assistance with representative payee services.
Provide training and onsite support to individuals who are interested in participating as Peer Support Specialists.
Enrollment into representative payee services through the Social Security Administration and Veterans Affairs for clients who need to utilize the service.
Management of this service on behalf of the client when appropriate.
Parenting education and family skills training provided to interested clients on an as-needed basis, and offered as a consistent skills group class for clients who need additional assistance in this area.
Referral to GPD and HCHV staff for high-acuity clients who need adult attendant day care.
Perform other duties and tasks as assigned by the Site Director or designee as appropriate.
Required Job Skills:
Ability to work independently, with minimal supervision while maintain an effective working relationship with other staff members.
Be detail oriented, able to set priorities, meet deadlines, maintain a professional attitude and conduct appearance at all times.
Able to set and maintain professional boundaries at all times with tenants adhering to agency policy and procedures.
Excellent interpersonal communication skills with the ability to interact with participants of varying ethnic, social or economic backgrounds.
Knowledge of mental health issues, drug and alcohol addiction desired.
Experience working with a homeless population highly desired along with a knowledge of veteran related issues.
Current working knowledge of community resources available to assist clients with achieving goals on their ITP.
Experience and knowledge of conducting individual case management and group sessions.
Proficient in office software including:
Word, Excel, Outlook and internet use.
Qualifications:
2 years' experience in case management and experience in homeless programs required.
AA degree or higher in human services, social work or psychology required.
Some experience in community outreach and available community services highly desirable.
Experience working in permanent supportive housing projects is desirable but not required.
Offering $24.
43 - $29.
57 per hour, DOE and location.
