The Village Case Manager works within a managed village or encampment, and assists clients with accessing services they need to secure their physical and mental well-being; helps clients prepare for and find housing; develops case management plans for assigned clients; monitors progress by meeting with clients regularly; works with other service providers to address the needs of clients; and documents all activities in a timely manner.
Supervision Received:
The Case Manager is directly supervised by the Program Manager.
Supervision Exercised:
The Case Manager position does not supervise other staff.
This position is responsible for working collaboratively with the Program Manager, site residents, other staff, and volunteers, as well as other service providers.
Essential Job Functions include, but are not limited to the following:
Meets with clients to complete the intake packet and/or uses the initial Goals and Plans as the basis for ongoing case work; conducts assessment of incoming clients to determine both strengths and needs; develops, implements, and monitors Individual Action Plans identifying goals for housing, income development, mental and physical health, self-care, legal issues, etc.
; coordinates with a team of village staff, service providers, and volunteers.
Meets weekly with clients to build trust; listens to their stories; works with clients to update goals; uses harm reduction strategies to encourage sobriety; helps clients focus on employment and housing readiness; assists with locating employment and housing.
Refers clients to appropriate local resources for benefits, substance abuse treatment, mental health counseling, etc.
; assists clients with accessing services; schedules appointments; assists with locating transportation; follows up on outcomes of appointments.
Completes accurate and timely Federally mandated data collection for the HMIS system and Coordinated Entry for each participant served; participates in and completes required trainings; documents all activities with and related to clients, including weekly meeting notes, visits to social service and other care providers; maintains confidentiality for residents and SAVS information.
Files and assists clients with filing written reports related to incidents involving villagers; reviews all incident and grievance reports and follows up on those that pertain to their clients; provides assistance in resolving conflicts and issues for all residents; manages conflict or difficult situations constructively and seeks appropriate assistance.
Attends and participates in Village Community Meetings and events; assists with discussion and resolution of Village concerns and issues, including those related to the Code of Conduct, visitor privileges, and site maintenance.
Monitors program policies and procedures.
Supports compliance with all organizational contracts and community agreements related to the site program.
Working Conditions and Physical Requirements:
The Case Manager works on-site and may walk/stand several hours each day on uneven and wet surfaces; may lift and carry objects weighing up to 30 pounds.
The Case Manager also works in an office setting, either on-site or remotely, sitting for several hours each day; types, writes, and uses communication devices.
Knowledge, Skills, and Abilities:
Knowledge of:
Case management; computer applications; HIPAA compliance; local homeless services and programs; conflict de-escalation techniques.
Ability to communicate effectively; work well with those from disparate backgrounds; make oral and written presentations to groups and organizations of various sizes; and use a computer for word processing and database management.
Required Minimum
Qualifications:
One year of case management experience or experience working or volunteering with individuals experiencing homelessness or other marginalized communities required.
Bilingual Spanish/English is required for some positions.
